INSTRUCTIONS: To sign up for CSA, print out this order form and the CSA agreement (link at left menu) Complete and mail your signed forms, along with $150.00 deposit. Make checks payable to Suzanne Ballard. Send in care of New Moon Farm, 1420 Belk Rd., Mt. Ulla, NC 28125.

Choose any combination of following options.
(Shares run for 30 weeks and price includes home delivery. Egg share runs for 20 weeks only.)
Basic Share (Produce only) - $30.00 per week
Medium Share (Produce only) - $42.50 per week
Family Share (Produce only) - $55.00 per week
Farm Goods Share - Add $5.00 per week to share
20-week Egg Share - Add $75.00 to share total

Payment Options:
Full Payment (Due by March 15, 2008)
Partial Payment
Share may be paid in 2 equal installments (after deposit is deducted) and requires pre-approval.
1st payment is due by March 15th, 2008 with remaining balance due by June 15th, 2008.
